Creating a Permittee Name Change Supplement Application
A name change supplement is necessary when an applicant team member changes, for instance, when a property owner changes while work is in process. Only internal PennDOT personnel can create a Name Change Supplement; however, the option is still available for selection by Applicants. An Applicant selecting to create a Name Change Supplement is informed that only PennDOT can do so and is provided with a link to the Name Change Form to be filled out and provided to PennDOT.
If you are an Applicant, there has been a change in Permittee and/or Co-applicant, and the permit is already closed out, you must still contact your District Permit Office.
To Apply for a Permittee Name Change Supplement:
1. Access the Application Information screen, Permit, License, or Supplement/Assignment Lists screen of the permit for which a name change supplement is needed.
2. Click the Create Supplements button.
Note: The Create Supplement menu button is hidden if supplements of all possible types are in process. If a change to work supplement is in process a user can still request a time extension, for example.
3. Select the Permittee Name Change option. The Create Supplement screen appears. In the header, the permit or license Application Number and the Permit or License Number display.
4. Enter the details of and the reason for the need to change the name(s) on the permit in the Applicant Supplement Description and Justification field.
5. Click Save & Exit. The Supplement Application Information screen appears, opened to the Setup>Information tab. While much of the information on this tab is for display-only, you can edit the information you entered on the Create Supplement screen, if needed.
6. In the Applicant Info section, click the Edit button. The Applicant Information page displays. At the top of the page, the section displays the information for the applicant/owner as it appears on the original permit application (or the most recently issued Name Change Supplement). The section displays the same information, but all fields are editable with the exception of Created By.
7. In the To section, change Applicant/Owner Information to the way it should appear on the supplement.
8. Click Save & Exit. The Supplement Application Information page redisplays with the new information for the Applicant/Owner displayed in the Applicant Info section.
9. Click the Edit button in the Co-applicant Firm Info section. The Co-applicant Information page displays with all the Co-applicant fields blank and editable.
10. Enter information for the new Co-applicant(s), as appropriate.
11. Click Save & Exit. The Supplement Application Information screen displays. The new Co-applicant(s) are listed in the Co-applicant Firm Info section.
12. If appropriate, delete any co-applicants that were part of the original application or a previously issued name change supplement by clicking on the Delete link on each row.
13. In the Applicant Contact Info section, click the Edit button.
14. Enter any additional relevant contact information for the new Applicant/Owner in the Applicant Contact Info field.
15. Click Save & Exit. The Supplement Application information screen displays with the text you just added in the Applicant Contact Info section.
16. Click Save & Next. The Setup>Fee information tab displays. Permittee Name Change Supplements are usually $10. The appropriate fee will be automatically listed on the Fee Information tab. However, this fee information is editable and you can remove it and/or add other fees as necessary in the same way you could add/edit fees on the permit application. 17. Click Save & Next. The Setup>Attachments tab displays. Attach necessary documentation (such as the deed) to the supplement application in the same way documents are attached on a permit application. See Attachments if you need help adding attachments.
18. If you want to see a list of all the documents attached to this supplement, the permit application, and/or any other supplements associated with the permit, click the Setup>Attachment History tab.
19. From the Setup>Attachment History tab, click Save & Next OR just click on the Setup>Supplement Summary tab, which displays in a read-only view, all the information entered for the supplement you are creating.
20. From the Setup>Supplement Summary tab, click another tab or the Supplement Application link.
21. When finished with the application, submit it to PennDOT by clicking the Submit button on the tool bar. A Submission Authorization Confirmation prompt displays.
22. Click an option and then click on the I Agree button. The Supplement Application Information page continues to display, but the status of the supplement is Submitted.
NOTE: Once a Permittee Name Change supplement is issued, the security for who can view/edit the application changes, as appropriate.
