Billing Account Information section
Customer Account Number	Link	Customer account number used for viewing or editing.
Customer Name	Link	Populated by user login or entered by Receiver. Can be updated by applicant, co-applicant, and engineering firm until Submit. Can be updated by Receiver until reviewers are assigned. No access by Reviewers after review is completed.
Billing Address Line 1	R	First line of billing street address.
Billing Address Line 2	R	Second line of Billing address.
System Type	R
Billing City	R	Billing city.
Billing State	R	Billing state.
Billing Zip Code	R	Billing Zip code.
Billing Contact Information section
Contact ID	R	Sequential numbering if more than one contact.
Contact Name	Link	Name of customer contact. Link accesses the contact for viewing or updating.
Contact Phone Number	R	Contact telephone number, with area code.
Contact Mobile Number	R	Contact mobile telephone number and area code, if applicable.
Contact Email Address	R	Contact email address.
Primary	R	Identifies contact as primary by displaying 'Yes.'
